- Call 312-436-1657, press 0 for the intake line, and leave a message with your contact information and a good time to reach you. Barring weekends, an intake specialist will return your call within 24hrs.
- Fill out the request form at the bottom of this webpage and someone will respond in similar fashion.
Insurance
All of our clinicians are in-network behavioral healthcare providers for BCBSIL PPO, Blue Choice PPO, and Professional Benefit Administrator insurance plans. Some are also paneled with Aetna PPO, UnitedHealthcare, and Medicare. We are considered out-of-network providers for all other insurance plans, and can help you navigate your plan’s billing processes. We offer competitive fees, and for those without health insurance, we’re happy to negotiate a fair rate for our services.

Location
CCA’s offices are conveniently located in the Garland Building at 111 North Wabash Avenue, Suite 1400. The building is ½ block west of the Chicago Cultural Center at the corner of E. Washington St. & N. Wabash Ave, and is easily accessible by public transportation. Valet parking is available at the building entrance, and limited street parking is available on Wabash Avenue.
First session & paperwork
Once an initial appointment is scheduled, please remember to bring your insurance card and ID with you. The initial meeting will last approximately one hour, during which you and your therapist will explore what brings you to therapy and the degree of therapeutic “fit.” There is a small amount of paperwork involved in this first meeting, some of which you may download here, complete, and bring with you. Your therapist will have forms available on-site as well, and will review all paperwork and related questions with you.
